I understand that the insulation in the atic or under the metal roof can reduce or eliminate the noise coming into the house, but what about outside, on a patio, or inside with the windows open?

I have never known a residential metal roof to have the "expected" metallic pinging sound -- like water dripping into a bucket. Yes, from a window, etc. you may hear a louder rumble than normal but certainly nothing I or any of my customers (or my wife!) have ever noted as being objectionable.
Also, the more textured the roof is, the quieter it will likely be. The stone coatings that are on some products also enhance quietness.
