Is there a way to tell if the fastener has been installed too tightly just by looking at it? How tight is too tight? Does the neoprene need to be cracked and/or squeezed out? Is just a dimpled surface too tight, especially if the metal is unsupported (no sheathing) underneath? Any references I can look at for this? The ones I found don't give the answers to these questions.

Thanks. So is any observation of a washer protruding up around the lip of the washer no good? What if it is just squeezed to the side?

If it is squeezed to the side, that typically indicates being installed out of level which is not ideal.
That said, the washers are designed to accommodate some less than perfectly perpendicular installation.
Biggest issue with exposed fasteners that we see is being installed way too tight.
